While, under normal circumstances, you do not need a Model Release to sell a picture of a recognisable person to a magazine for editorial use, you will need one if you hope to sell one for advertising. And, if you contribute to a photo library, it will want to know whether you hold a release for the people pictures that you lodge with them.
A Model Release is simply a document that the person you have photographed has signed (or their parent or guardian if they are a minor) which gives you permission to use the picture in any way you see fit subject to any exceptions included on the form such as not using the model's name. There are a number of different wordings used and some are long and some are short, some are in simple language and others are in full-blown legalese but basically they all do the same thing which is give you permission to use the image. The Bureau of Freelance Photographers used to sell pads of model release forms but no longer does so because it is so easy to go to the web and search for 'Model release' and download a form or make your own after reviewing existing forms.
Below is one of the simplest forms that the New York Institute of Photography has on its website with permission for it to be downloaded and used subject to you not holding them responsible for any dispute that may arise. It is simple but it covers the important points and is not likely to create a lack of understanding by the people you are asking to sign it. It will also fit easily onto an index card so is convenient to carry with you.
